Note: All Affidavits must be sworn in front of a Commissioner of Taking Affidavits. Please see the Financial Aid office to have your affidavit sworn. Out-of-town students will find a Commissioner at their local court house.

- Part-Time Student Loan and/or Canada Study Grant Application
- Bursary for Student with Disabilities Application
- Child Care Bursary Application Form
- Continuation of Interest Free Status
If you are returning to school full time, but will not be receiving new OSAP funding, you will need to advise the National Student Loans Service Centre that you are once again a full time student and want to return your loans to Interest Free Status. Print, then complete, the "Continuation of Interest Free Status" form and submit to the Financial Aid Office. Please click on Continuation of Interest Free Status to be brought to the OSAP website and then search for the form by the form name.
If you are having difficulty repaying your student loans, a number of options are available to you. Please refer to the CanLearn website or call the National Student Loans Service Centre at 1-888-815-4514 for assistance.
