- You are asked to hand deliver, mail, email, or fax your completed Confidential Student Health Record to the Health Centre by the end of August 2012. The fax number to the Health Centre is (807) 622-2646.

- The Thunder Bay District Health Unit provides a “Drop-In” Immunization Clinic every Wednesday afternoons from 1:30 – 6:00 PM (this clinic is for Measles, Mumps, Rubella, and Tetanus Diphtheria). The Thunder Bay District Health Unit does not provide TB Skin Testing (you will need to make an appointment with your family Physician or Nurse Practitioner). Please remember to bring your yellow Immunization Status Card to the Immunization Clinic. Please insist on receiving written verification of your TB status and show this written form to the Nurse at the Health Centre. - Out of town students must obtain their immunization status from their local district.
- Physicians, Nurse Practitioners, or Walk-In Clinics throughout the city may be helpful.
- If you are having difficulty obtaining your immunization records, please call the Thunder Bay District Health Unit at 625-5971, or your local health unit if you live outside of Thunder Bay.
First Aid - CPR Requirements Criminal Record Check
First Aid and CPR Certificate:
Personal Support Worker students must provide proof of current certification in the First Aid and CPR (Basic Rescuer Level C) course. Students must submit a photocopy of their certificate on the first day of classes.
Criminal Record Check (Vulnerable Screen):
To be eligible for participation in the first semester field placement of the Personal Support Worker program starting, a student must not have been convicted of any criminal offence for which the student has not yet been pardoned. An unpardoned criminal record could result in the student not being able to participate in field placement courses and therefore prevent the student from graduating. A recent and valid criminal reference check (CRC) is therefore required to enable the student to progress through this program.
Note: Procedures and regulations for obtaining criminal record search documents are controlled by the RCMP and the Government of Canada. These procedures and regulations may change at any time without notification. Confederation College will endeavour to notify students of any changes when they come to our attention. As of March 1, 2011 the following reflects the current procedures and regulations of the RCMP with regard to Vulnerable Sector Verifications.
The RCMP identified a means to enhance vulnerable sector checks to make the searches more rigorous. In the interest of public safety, the enhancements were quickly implemented to protect vulnerable members of society. As a result of the enhancement, a Vulnerable Sector Verification now requires the submission of fingerprints whenever the gender and date of birth of an applicant match to an existing pardoned sex offender record. This is not an accusation of criminality, but a requirement for a thorough verification to confirm identity and protect personal privacy. This process will take at least four months.
Any costs associated with acquiring a Criminal Reference Check (CRC) will be the responsibility of the student applicant.
Sincerely,
Debra Walker
